IntroductionThe Cardiac Rehabilitation service offers an in-depth, individualised programme of care, which includes education; advice; monitoring and a variety of exercise programmes to enable patients and their families to regain their confidence, and resume as near normal a life within the community. Patients are able to pick and choose their options in order to meet the goals they set. Our patient groups include Heart Failure, Myocardial Infarction, Cardiac Surgery, Angioplasty; we also have patients who have had Cardiac Transplant, Angina and Internal Defibrillators implanted through the service, although these are less routine. Patients are mainly referred from the nursing staff on the wards, but we also have referrals from other Hospitals, GP's, Practice Nurses and Community Matrons. The Cardiac Rehabilitation team is made up of Cardiac Specialist Nurses, Cardiac Exercise Instructor, Secretary and Data Input Clerk. We have excellent working partnerships with North Lincolnshire Council, which has enabled a continued exercise programme to be established, this ensures a smooth transition for patients to continue their exercise routine, and at this time partners often join in the exercise. We encourage a range of students and professionals to spend time with us, in order for them to gain a clear insight as to what the service has to offer and how they are able to use us as a valuable resource. We recognise the importance of high quality care and strive to maintain and share good practice; we achieve this through networking with other teams, working closely with with our Cardiac and Stroke network and the NHS Improvement Team. |
