Macmillan health care team

Our Macmillan health care team provide care to patients who are at the end of their life and wish to stay at home.

The team take referrals from healthcare professionals for terminally ill patients who have chosen to die at home rather than in hospital and therefore need additional support and assistance to be able to do this.

Many of our patients would rather die at home surrounded by their family and friends and their home comforts than in hospital. Our staff are here to ensure their their wishes are respected and anything we can do to make their final days more comfortable can make a big difference.

Referrals are taken from staff in hospitals, out in the community and GPs. As the team develops it is envisaged that staff will also be able to provide additional assistance to people who have chosen to receive end of life care in a care home setting.

All the nurses have been so kind, professional and helpful, offering lots of welcome advice. They have treated my husband with the utmost kindness and respect and all of them make up an absolutely amazing team.

My future care plan

‘My future care plan’ has been designed by patients and health care staff to help people to put their affairs in order. It helps patients think about the type of care they wish to receive in their final days. This could be decisions about whether they would want to be cared for in their own home or in hospital, who they would like to make decisions for them if they are unable to and what kind of funeral they would like. Although aimed at end of life patients the document can be used by anyone to document their wishes.
