Introduction
The purpose of the Rehabilitation Medicine Service is to provide a co-ordinated approach to
rehabilitation, which enables physically disabled people live in their preferred environment with the best
possible quality of life. The team serves the population of North and North East Lincolnshire aged between
16-65 with a physical disability.
Objectives
-
To provide a community orientated service in the location/environment
most appropriate for and, where possible, preferred by the client; including the patient’s home, day centres,
nursing/residential homes, hospital ward.

To provide a consultant-led specialist service which has the
confidence and expertise to manage people with severe disabilities in the local environment thereby reducing
the need to move away from home.

To promote long-term management through on-going assessment planned
intervention and life-planning thereby avoiding such crises as medical complications, hospital admissions and
breakdown of carer support.

To provide a service, which works in partnership with other agencies,
particularly Social Services, to ensure that health and social input, is part of a co-ordinated approach.

To provide a single source of information for the patient and their
family.

To operate an identified pathway of care and review process.

To give the lead to the patient and carer in managing their own care
plan.

To provide a service which is sustainable in terms of expertise,
funding, recruitment and retention.
