What we do
Rehabilitation is based on a person centred approach and focuses on supporting patients and their families, with an individual rehabilitation plan, designed and developed to meet the diverse and changing needs that present post injury or illness.
The centre is led by a Consultant in Rehabilitation Medicine which enables us to provide services for individuals who require a high level of nursing and medical input e.g. who have a tracheostomy; require specialist treatment for contractures including botulinum toxin, serial casting and thermoplastic splinting, or who are in states of minimal consciousness.
Evidence-based clinical outcome measures are used to assess and monitor the progress of our patients – these include Functional Independence Measure/ Functional Assessment Measure (FIM/FAM), Goal Attainment Scale (GAS), Rehabilitation Complexity Scale (RCS), Northwick Park Dependency Scale (NPDS), Northwick Park Care Needs Assessment (NPCNA) to the Modified Overt Aggression Scale (MOAS). We can undertake the Sensory Modality Assessment and Rehabilitation Technique (SMART), and similar assessments for those in a minimally conscious states.
We are very proud of our Person-Centred Rehabilitation process, and we adapt our Rehabilitation Model to each individual.
