The CDC at Grimsby is based on the hospital site next to the children and family services building.
Within the CDC at Grimsby we have a sensory room, splash pool, soft play room including a ball pool and therapy rooms.
Referrals
Referral for a multi-agency assessment is usually via the early support pathway, on completion of an access pathway form by professionals in liaison with parents/carers.
If you have concerns about a young child’s development and you need advice about referral, assessment or ongoing support, please contact (with parental permission) one of our centres.
