
NHS Humber Health Partnership works in collaboration with Patients Know Best (PKB) to provide an online service that gives you secure access to your personal health record.
What is Patients Know Best (PKB)?
PKB is a free online patient portal that allows you to store your health records in one place to give you instant access to and better control of important information such as your appointments, care plans and medical correspondence between you and your healthcare team.
You can also share your health information with healthcare professionals, family and carers – any time, anywhere.
Building an online record of your health can help keep you and those involved in your care informed more regularly, which can lead to better decisions about your health and the care that you need.
PKB is also connected to the NHS App, where you can access other information, including from your GP. The PKB step-by-step guide can help you register using your NHS login.
If you’ve received an email which says “Hull University Teaching Hospitals NHS Trust or Northern Lincolnshire and Goole NHS Foundation Trust has connected your Patients Know Best (PKB) record to a new team that is, or has been, working with you. The team’s name is the Patient Records Team” please be assured, it is genuine.
What is available?
Some of the features within the portal are not currently available in all our specialties, but we are continuing to work with our teams and PKB to be able to offer our patients a greater range of services. Updates regarding new developments will be provided here and via our social media channels.
What is available for patients being treated at Hull University Teaching Hospitals NHS Trust (HUTH)?

Benefits
PKB is free to use and will allow you to:
- See your record anytime: See all your appointments, in one place, wherever you go.
- Receive notifications when new information is available, prepare any questions ahead of appointments and manage your health and care in the meantime.
- Add your data: Add important information about your past medical history or any old clinical documentation so it’s there in one place, when you need it.
- Track your health: Keep a journal, monitor your symptoms and add measurements. Understand what’s normal for you. Then, if something isn’t quite right, you know when to contact your health and care team.
- Share your record: Instantly share all, or parts of your record with anyone who needs it. Quickly give access to your GP, hospital consultant, at-home care provider, pharmacist, paramedic, carer or next of kin. Control what they see with granular privacy and consent settings and best of all, access it wherever you go (at home or abroad) for real peace of mind.

Add your GP data to your PKB record

If you’re an adult and your NHS GP is in England, you can now store a copy of your GP data into your PKB record, adding to information you already access from the Trust. This creates a more complete personal health record that you can use to manage your own health and securely share with others involved in your care.
Initially, your GP data will include diagnoses, medications and appointments, with more data coming later.
To add your GP data, simply log into PKB using your NHS login and PKB will guide you through the process, including obtaining your permission. Your GP doesn’t need to do anything – PKB is simply storing data you already see in the NHS App. You can get more information from this video, visit Add your GP data to your PKB record – Patients Know Best or email [email protected].
