NHS Humber Health Partnership
Hull University Teaching Hospitals NHS Trust and Northern Lincolnshire and Goole NHS Foundation Trust are working together under the group name of NHS Humber Health Partnership. This page is relevant for patients of both Trusts.
NHS Humber Health Partnership is working with Isla Health, a digital pathway platform that allows patients to securely share information with their clinical team, including photos, videos, questionnaires and sound recordings.

How it works
1. You’ll get a secure link. We will get in touch with you via text message or email, containing a link from Isla.
2. You’ll share information. This link will ask you to submit information requested by your clinical team, which could be a questionnaire, image, video, or sound recording. You don’t need to download an app or register, but you will need to confirm some details so we know it’s you.
3. Your team reviews it. Once submitted, your clinical team can then review the information to support your care delivery.
This will help your team assess your condition, and help ensure you get the right care at the right time.

What are digital pathways?
Digital pathways are essential for the NHS because they offer a practical solution to the current system’s inefficiencies. The healthcare system is burdened by time-consuming administrative tasks, duplication, and disconnected tools, all of which take away from valuable patient time. Digital pathways tackle these issues directly by using remote information capture, asynchronous communication, and automation for predictable tasks like triage and monitoring. The aim isn’t to replace clinical interactions but to streamline processes, thereby unlocking capacity, reducing delays, and restoring valuable time to care. This results in a more consistent, efficient, and scalable care model that benefits patients, clinicians, and the entire health system.
