What we do
We’re here to reduce the distress caused by a life-limiting diagnosis by offering specialist advice and symptom management as well as emotional and psychological support.
Macmillan nurses provide direct and indirect support to both patients and other professionals such as district nurses and community nurses and become involved with patients with extraordinary need. This often reflects a complexity of problems across the physical, psychological, social or spiritual domains.
Each specialist Macmillan nurse within the team has their own caseload of patients and they support these patients and their families in their own homes whilst maintaining very close links with the district nursing service, the local hospice and the hospital based Macmillan nurses.
Communication with patients and their relatives will often be a combination of face-to-face visits within their own home and telephone consultations where appropriate. Once symptoms and complexity are under control then the Palliative nurses will conclude their involvement.
It’s a seven day a week service with office hours 9am to 4.30pm, with a reduced service available at weekends and bank holidays to respond to urgent enquiries.
We work with patients who are registered with a GP under the North Lincolnshire Clinical Commissioning Group.
