We take referrals from healthcare professionals for terminally ill patients who have chosen to die at home rather than in hospital and need additional support and assistance to be able to do this.
Many of our patients would rather die at home surrounded by their family and friends and their home comforts than in hospital. Our staff are here to ensure their wishes are respected and anything we can do to make their final days more comfortable can make a big difference.
Referrals are taken from staff in hospitals, out in the community and from GPs. As the team develops it is envisaged that staff will also be able to provide additional assistance to people who have chosen to receive end of life care in a care home setting.
All the nurses have been so kind, professional and helpful, offering lots of welcome advice. They have treated my husband with the utmost kindness and respect and all of them make up an absolutely amazing team.
